Why freezing of gait causes so many falls

Freezing of gait is a brief, involuntary halt in movement — the feeling that your feet are stuck to the floor even though you intend to walk. It tends to strike at transitions: starting to walk, turning, passing through a doorway, or approaching a destination. Because the upper body often keeps its forward momentum while the feet stop, freezing is a powerful fall trigger. The response gap when you live alone

A freeze-triggered fall carries the same underlying risk as any solo fall: the danger is often the time on the floor afterwards. Most falls happen when the person is alone — 82% in a study of adults 90+, of whom 80% could not get up unaided (Fleming & Brayne, BMJ 2008). And time to discovery drives the outcome: about half of older home-fallers who lay on the floor for more than an hour died within six months (Wild et al., BMJ 1981), with mortality of 12% within an hour rising to 67% after more than 72 hours (Gurley et al., NEJM 1996). So while freezing can't always be prevented, the wait that makes a fall dangerous can be shortened — and that is the lever a safety net pulls.
